Breast Reshaping (Dr. Friedman only)

The most frequently performed cosmetic surgery procedure in the U.S., breast augmentation can give women with small or unevenly sized breasts a fuller, firmer, better-proportioned look through the placement of implants in the breast. Women may elect to undergo breast augmentation for many different medical and aesthetic motivations, including balancing breast size and compensating for reduced breast mass after pregnancy or surgery. The procedure may be combined with others such as a breast lift for more satisfying results.

Implants are silicone shells filled with saline (salt water) or “silicon gel” and are placed behind each breast, underneath breast tissue or pectoralis muscle. We specialize in the placement of subfascial plane, the space under the frontal covering sheath of pectoralis muscle. The procedure lasts one to two hours and is typically performed with intravenous sedation, although local anesthesia combined with a sedative is also possible. After surgery the patient’s bustline may be increased by one or more cup sizes.

Incisions are made in inconspicuous places to minimize scar visibility (in the armpit, in the crease on the underside of the breast, or around the areola, the dark skin around the nipple). The breast is then lifted, creating a pocket into which the implant is inserted.

Placement behind the chest wall muscle offers a few advantages over placement beneath the breast tissue only. These include reduced risk of capsular contracture (post-operative tightening around the implant) and less interference with mammogram examinations. Subfascial placement would minimize displacement of breast implant by Pectoralis muscle contraction in the case of subpectoral muscle placement. It also has thicker breast implant coverage to give a more natural contour.

After the implants are placed and centered beneath the nipples, incisions are stitched, taped and bandaged. In a few days these bandages may be replaced with a surgical bra. Most patients feel tired and sore after surgery, but this usually passes in a day or two and many patients return to work within the week. Stitches are removed in a week to 10 days and any post-operative pain, swelling and sensitivity will diminish over the first few weeks. Scars will begin to fade in a few months and will continue to fade for months or years.

Fat transfer breast Augmentation has been a “New kid on the block” especially with new enhancement agents to increase its fat survival rate, and is now acceptable as well for breast enhancement techniques in over 50% of Asian women.

Breast Lift (Mastopexy) & Reduction

With gravity and age, supportive ligaments and parenclyma of breast weakness and stretches, leading to ptotic breasts. Depending on the size, such breast can be lifted, or reduced and nipples lifted to the “proper” height. Scars are designed to be hidden around the areola, under the breasts, or at times, in the lower frontal surface. Occasionally, drain may be needed. Such out-patient surgery last from 1-3 hours and needs 1-5 days at home. Pain, amazingly is minimal.

Liposuction, with or without Laser Enhancement (Dr. Rassel only)

One of the most popular cosmetic surgery procedures, liposuction sculpts the body by removing undesirable fat from areas such as the abdomen, waist, hips, buttocks, thighs, knees, chin, jowl, neck and upper arms that have not responded to diet and exercise. Local anesthesia or general anesthesia may be used and the procedure can last anywhere from one to five hours depending on the amount of fat being removed.

There are several different methods Dr. Rassel uses when performing liposuction, including tumescent liposuction and Laser assisted liposuction. In all types, a tiny incision is made in an inconspicuous place, and an anesthetic solution is put into the fatty tissue. Laser fiber cannula is then inserted to both dissolve the fat and tighten skin dermis. Then a small cannula is inserted through the same tiny incision into the fat layer and suctions out oils and fat globules.

Recovery from liposuction is relatively quick and most patients feel up to returning to light duty within 2-7 days. However, compression garment is recommended for 4-6 weeks.

Abdominoplasty (Tummy Tuck) (Dr. Rassel only)

A “Tummy Tuck” or Abdominoplasty is a surgical procedure designed to flatten a protruding abdomen by tightening the muscles in the abdominal wall and removing any excess fatty tissue and skin.  Many men and women successfully have this procedure that helps to improve their self esteem.  Realistic expectations are needed to appreciate any probable surgical outcome.

The best candidates for this procedure are those whose weight has been stabilized after their weight loss program or those who feel they are at their ideal weight.  Sometimes, the tummy tuck is combined with other cosmetic procedures like breast augmentation and liposuction.

The tummy tuck is usually performed in conjunction with liposuction of the upper abdomen and flanks.  An incision is made in the bikini area and sometimes an incision up to the belly button.  Excess skin and fat will removed through this incision.  The new flat abdominal skin will then be sutured or stapled after anchoring down to the low pelvis area.

After the procedure, it is important that the patient have bed rest.  Most patients are back to work in 1-2 weeks.  Results can be dramatic and patients love their new figure!

Fat Transfer

Also called micro-lipoinjection or fat transplantation, autologous fat transfer involves the removal and relocation of the patient’s own body fat to plump up an area that has lost volume as a result of surgery, injury, aging or another cause. Fat is removed through liposuction, often from the abdomen, buttocks or thighs. It is then processed and injected into the area being treated. Both sites are numbed with local anesthesia.

The procedure is commonly used for breast augmentation, buttock enhancement, facial wrinkle, and crease reduction. Because the transplanted fat comes from the patient, it does not carry a risk of allergic reaction or rejection. Other advantages include a short recovery time, long-lasting results and effectiveness on any skin type. New studies seemed to indicate that enhancing agents such as PRP or stem cell extraction are exciting frontiers in increasing fat cell survival after transfer.
